Thomas Collentine died peacefully on September 4, 2011 surrounded by his wife Monique. Tom had a 5-year battle with cancer. Tom and Monique resided in Sarasota FL which also was Tom’s business location.

Thomas Richard "Tom" Collentine, 54, of Lakewood Ranch, formerly of St. Petersburg, Crystal Lake, Ill and Eljin, Ill. died Sept. 5, 2011.
A Celebration of Life service will be held at 6:30 p.m. Monday at Bayside Community Church, 15800 State Road 64, Bradenton.
Survivors include his wife, Monique R.; his brother, Mike and his mother, Elizabeth J.
